Subsidy Use:

• I understand that the shoe subsidy, valued at up to $150, is to be used exclusively through the company-approved online vendor for the purchase of safety-toed shoes.
• I acknowledge that the cost of the shoes will be directly charged to the company and that reimbursement is not required.

• I understand that if I voluntarily resign or am terminated within six (6) months of purchase, I will be required to repay the company for the cost of the subsidy ($150).
• I agree that the company may deduct this amount from my final paycheck, in accordance with applicable labor laws.

• I understand that wearing safety-toed shoes at all times while on the manufacturing floor is a condition of my employment.

• I acknowledge that the payback requirement may be waived at the discretion of the company for involuntary separations not related to performance or conduct.
